Erythrinus erythrinus, approximately 16cm in length, and showing far more and far, far more intense red than the picture suggests. As a bonus, he is taking frozen krill and silversides readily.

As much as 22cm and pretty voracious.

No idea...probably less than 12 months.

No, ordered him (and 5 others) into our shop (Riverfront Aquariums in Calgary) from a South American distributor we buy from.

Not so bad...he is merrily co-habitating with Polypterus, Ctenopoma, Xenomystus, and Anabas right now.
